Completion of this form is in leu of emailing a completed printed form to Carolina Schottland, Lake County Extension Foundation Board President, at Carolina@lcextensionfoundation.com or dropping off a completed copy at the Lake County Extension Office.

Pre-Event materials will be updated until 2 weeks before the event so the earlier you get us your name and logo, the more exposure you will receive!
